Please fill in all entries below and submit your online reservation form before the deadline (Tue, Jul 16). Each event reservation submission is limited to a single name. You will receive an automated email confirmation of your reservation request, assuming that you enter your email address correctly. After clicking "Submit Form", your reservation data will be displayed on a summary web page that you must print and mail in along with your check. The mailing address will be shown on the web page after you click "Submit Form". Your reservation is not complete until we receive your check.
Cost for this event is $10 members / $10 students with school ID / $20 guests. Please make checks payable to "IEEE-PES Chicago Chapter". Once your check has been received, an email notification will be sent to your email address informing you of the status of your reservation request, either "accepted" or "waitlisted". The event is limited to 25 people. Note: all participants must be at least 21 years of age and provide an ID at the door.
